Computer / Database Problems

If you are having a problem with the library's computer systems such as the UT Library Catalog, OhioLINK resources, or the Research Databases, please fill out the Problem Reporting Form.

Collection Management

• Suggest materials for purchase.  - Please see the Collection Management page for guidelines. 

Circulation & Course Reserves Forms

• Course Reserves Form (.doc) - To place items on reserve for a class (for instructors), see the Course Reserve Services for Faculty page for more details.
• Proxy Borrower form (.pdf) - To designate an authorized proxy borrower who can use your ID card to check out items.

Interlibrary Loan Forms for requesting items not available in UT Libraries collections

• ILL Loan Request for Books, Microfilm, or Audiovisual Items
• ILL Photocopy Request for Articles or Chapters
• Depository Photocopy Request for items in the Northwest Ohio Book Depository.

Reference & Instruction Forms

• Ask a Reference Question.  (Please see the Ask A Librarian page for other options for reference assistance which may provide a quicker response.)
• Ask a Reference Quesion on Health Science Campus
• To schedule a Library or Information Literacy Instruction session for your class (for instructors) on the Main Campus or Scott Park Campus.
• To schedule an Individual Instruction Appointment for one-on-one assistance with a reference librarian, please e-mail the Information & Instruction Services Division, Kathy.Hall@utoledo.edu.

Assistance for Patrons with Disabilities

Users with disabilities may complete these forms and return them to the Circulation Desk to have materials retrieved from our collections or photocopied at cost.  Please see the Services for Patrons with Disabilities page for details or alternate methods of requesting assistance.
